Hello! I am excited to have you for your virtual appointment at the 108. I want you to have the best experience, so we recommend finding a quiet and comfortable place where you won’t be disturbed. Keep in mind that a virtual healthcare appointment is just as effective as an in-person visit specially for mental health.

During your virtual visit, we will discuss your symptoms, medical history, and recommend next steps. If you need further exams or diagnostic tests, I will provide you with directions on how you obtain them. Just like an in-person visit, you can ask questions and express your concerns in real-time. I hope this consultation will be efficient and helpful for you. I am looking forward to speaking with you.

Initial Psychiatric Evaluations for new clients: $150.00-$200.00 

Former patients: $100.00 

This typically takes between 45 min to 90 minutes depending on complexity.  The appointment includes an evaluation and treatment planning. This appointment also includes medication management, labs etc. 

Psychiatric Evaluation

Follow up Medication Management: $75.00-$100.00

Typically takes between 15-30 minutes depending on complexity. May include 15 minute psychoeducation and psychotherapy/coaching session. 



Medication Management

 The pursuit of attaining inner harmony and contentment is indeed a noble endeavor, and one that you are undoubtedly capable of achieving. My role,  is to intently listen to your personal narrative and equip you with efficacious tools that will enable you to decipher the intricacies of your thoughts and behaviors and ultimately overcome any negative patterns. My therapeutic approach constitutes an amalgamation of several empirically supported modalities namely: Cognitive Behavioral Therapy (CBT), Spiritual Psychotherapy, Buddhist Psychology and Interpersonal Therapy. Through these modalities, we can effectively bring about significant positive change in your life. It is imperative that you remain steadfast in your determination for self-improvement, and with my support, we can effectively unlock your true potential and facilitate a positive transformation that aligns with your goals.

Sessions: $50.00-$100.00 

Appointments are between 30 minutes to 60 minutes. Can be included with med management appointments. 


Ensuring optimal mental health necessitates a holistic approach. No matter how effective a medication regimen may be, suboptimal sleep, nutrition, or exercise habits can mitigate its efficacy. However, changing deeply ingrained habits can be challenging. As a qualified professional, I can provide guidance and support in taking small yet consequential steps towards transforming your habits for the better. By doing so, you can achieve significant positive outcomes that benefit your mental health and well-being.


Therapy can be either Coaching or Psychotherapy depending on the client’s needs.  Sessions are between 30 min to 60 min. 

Life Coaching

I am confident that I can assist you in learning how to incorporate meditation and mindfulness practices into your daily routine, allowing you to effectively manage your emotions. Working on the body part that stores trauma is essential for overall healing. Our body work therapist is akin to and is intuitive to identify the areas where energy is trapped causing mental health symptoms and are precursors to autoimmune and chronic diseases.

Range: $120 for initial assessment

$50 to $100 for Sessions between 30 minutes to 60 minutes. 

Initial Somatic Intake Assessment takes between 60-90 minutes.  

Somatic Therapy

Controlled Substance Policy:

New clients who are already on controlled substances and wish to continue on controlled substances, have to be seen in person. Our controlled substance policy and agreement form has to be signed by clients. 

Psychostimulants require the following:

  •  EKG and/or clearance from your PCP. 
  • If you have previously been diagnosed and treated for ADHD please bring your medical records with you. 
Your initial prescription will be included in your initial evaluation.
Med refills will be done monthly, and virtually as determined by your provider for a $50.00 fee/refill. 

Note: Per DEA regulations, patients who are prescribed a controlled substance via telemedicine will be required to abide by DEA rules and regulations. Clients will be updated with these with every appointment. 

Psychiatric Evaluations With Letter Only no Medication Management. 

This appointments requires two sessions and payment is required prior to second appointment

Services Provided for Established Clients Outside of Regular Appointments

– Emergency Medication Refill For Non Controlled Substances, 30 Day Supply:  $30.00

– Copy Of Records To Patient: $25.00 Requires Brief Consultation Prior To Release

– Review of Genetic Testing Results with Medication Review outside of client’s regular appointment: $75.00

– Emergency Calls, consults: $25.00 per 15 minutes

Educational Programs for the community $20.00 per person for a 30 minute virtual group session. Minimum of 5 attendees per session.

Functional Nutritional Evaluation

As a professional in the field, it is important to take into consideration all aspects of your health and nutrient levels. It has often been noted that conventional lab testing may not provide a comprehensive outlook on your functional nutrient needs. Through the use of cutting-edge laboratory testing technology, we can further examine your nutrient and organic acid levels, essential and metabolic fatty acid levels, amino acid levels, level of oxidative stress, and presence of toxins. This analysis will enable us to further optimize your treatment regimen and ensure that all your nutrient requirements have been met. Hormonal Testing

Adrenal burnout and fatigue pose one of the most significant challenges in modern society. The assessment of adrenal function is crucial for accurate diagnosis and treatment of chronic fatigue. The application of advanced salivary cortisol and melatonin testing can help evaluate the role of adrenal hormones in chronic fatigue. Tests involving saliva collection can provide non-invasive and accurate analysis of hormone concentrations. Precision measurements of cortisol and melatonin levels in saliva can be essential tools in identifying adrenal insufficiency or overactivity. Hence, salivary hormonal testing can be a key component in the modern management of chronic fatigue.

The examination of our intestinal microbiota and its correlation with mental health has led to a significant breakthrough in the medical field. Through my services, I am equipped to conduct extensive gastrointestinal stool evaluations, which allow for the identification of maldigestion, inflammation, dysbiosis, metabolite imbalance, and infection. Food and Allergy Testing

The food we consume significantly influences our emotional state, and some individuals have undetected dietary sensitivities that affect their mood. We can conduct innovative food sensitivity and allergy testing and recommend appropriate nutritional adjustments to optimize overall health.

Genetic Testing

The field of genetics presents a promising avenue for optimizing medication regimens and enhancing overall wellness. Current research suggests that individual differences in genetic predisposition can impact the efficacy of treatment. With the aid of genetic testing, it is possible to identify predispositions for certain illnesses and predict individual response to specific medications. This information can then be used to develop an optimal medication regimen that is tailored to the unique genetic makeup of each individual. Trauma Thought Processing

This is a necessary part of post  therapeutic applications of psychedelics, such as psilocybin, LSD, ketamine, and MDMA, show much potential in the treatment of several mental health issues, particularly depression and PTSD. One of the challenges of using psychedelics as therapy is integrating the insights garnered from these experiences into one’s daily life. However, with appropriate guidance, it is possible to integrate and process these experiences, resulting in meaningful changes in an individual’s thinking patterns. My expertise lies in helping individuals process and make sense of their psychedelic experiences and use them to develop new ways of thinking that can enhance their sense of purpose, meaning, and fulfillment in life. For clients who have a history of several failed medication trials, and have unclear multiple diagnosis in the past, there are several diagnostic tests and treatment options that will eventually be available at Empowered Choices: 

– Comprehensive Functional and Neurobiological review of symptoms.  

– Functional Lab testing (endocrine function: thyroid, adrenals etc; evaluation of inflammatory factors; and GUT  dysfunction. 

– Recommendations of appropriate treatment options: dietary supplements, body work etc.

– Referral to specialists as needed.

We are not in network with any insurance.  Our patients pay out of pocket. Depending on your out of network insurance benefits, you may be reimbursed by your insurance. Some may be reimbursed at 100%, or none at all.  Some will reimburse a certain % after meeting your deductible.

Do I have out of network coverage for psychiatry? If so is there a deductible?

If there is a deductible, have I met it? 

What % of fees will be covered? 

What is the allowable amount for codes:

We will give you an invoice after your appointment so you have your insurance reimburse you. 

You have a variety of options to pay for the service that you need. You can pay using cash, check, credit card, or even a cash app.

Another option is to open a flexible spending account with your employer. It is also called a pre paid medical account. You can use the money in the account to get reimbursed for uncovered medical or mental health expense. 

Unreimbursed medical/mental health expense can  also be a tax write off.

Almost everyone has insurance. Why did we decide not to take insurance? 

As a provider I realized that the most important thing in mental health as opposed to medical issues is developing a trusting relationship with my patients. 

This relationship is the foundation of your healing and the quality of care that I strive to provide for you. 

  1. The first important reason is that insurance companies, by deciding for you as a patient which treatments you will be reimbursed for, interject themselves between a doctor and patient. For my patients, I want you to have full access to the vast array of safe, effective treatments available
    and for us to decide based on your health needs what combination of different treatments will optimize your overall mental wellness.
  2. In order for a clinic to get reimbursed for your care, we have to employ staff to take care of billing, coding etc. Because our clinic doesn’t have a full office staff, our rates are a lot cheaper than other cash pay clinics or even cheaper than your accumulated insurance deductible. 
  3. In a traditional clinic, have you noticed your provider trying to get escort you out of the office or cutting you short during an appointment? At Empowered Choices, we are transparent with our services, how long the appointment is and how much they cost. I do not want to treat people like they are in an assembly line. 
  4. Finally, what is your mental health, happiness and success worth to you? In our clinic you have a say in your care. You can set upon an appointment at anytime that the clinician has a opening, 
  5. We will be offering membership packages that would allow you to avail of therapy at a lower rate.

We are a virtual clinic. At this time we only see clients virtually using an online platform called Simple Practice. You can use your phone, iPad or PC to connect with us. 

We understand that in person appointments are more comfortable for most. However, we are trying our best to lower the cost to you. A brick and mortar, staff, utilities cost the provider overhead see and the  pressure to make money to pay for all these expense. With financial gain in mind, the provider is forced to get clients in and out the door. 

My sole purpose is to offer mental health and wellness services that are affordable, and accessible. Our appointments are just as good as an in person appointment,
